When Can I Get Vaccinated?
While the vaccine supply is limited, COVID-19 vaccine will be offered in phases.
People who are more likely to have severe COVID-19 illness and workers that are
essential to the functioning of society will be offered the vaccine in the earlier
phases.
Vaccinations are not approved for those who are 15 years of age or younger. If
you are 15 years of age or younger, there is no need to answer any of the
questions asked in this vaccine eligibility questionnaire.
If you are between the ages of 16 and 64, please answer the following questions
to determine which vaccination phase you fall under.
If you are 65 years of age or older you are eligible for phase 1b of Virginia’s
COVID-19 Vaccine Distribution Plan unless you answer yes to questions 1 or 2.
After you have determined what vaccination phase you are eligible for, you can
find information on how to register to be vaccinated by contacting or visiting the
website of the local health department in the locality where you live or work.

If you are eligible for vaccination because of your occupation, please contact the
local health district where you work.
If you are eligible for vaccination because of your age or underlying medical
condition, please contact the local health district where you live.

Homes for the Aged
Intellectual and Developmental Disabilities Residential Center
Group Home
State Veterans Homes
Family Care Homes
If you answered yes to living in one of these types of long-term care facilities, you
are eligible for Phase 1 A of Virginia’s COVID-19 Vaccination Campaign. There is no
need to answer any further questions.
If you do not live or work in one of these types of long-term care facilities,
continue to question 2.
Question 2
Are you one of these types of healthcare workers?
Behavioral health providers
Community health workers
Dental assistant, hygienist or dentist
EMT or a first responder serving as an EMT extender
Environmental services worker
Healthcare trainee or volunteer
Home health worker
Human service provider
Infection control and quality assurance professional
Laboratorian
Mortician/funeral home staff
Nurse, nursing assistant or nurse practitioner
Optometrist
Personal care aide
Pharmacist, pharmacy tech or pharmacy workers
Physician or physician assistant
Public-facing public health worker
Radiological or other diagnostic/therapeutic tech
Respiratory, physical, speech or occupational therapist
Social worker
If you answered yes to being one of these types of healthcare workers, you are
eligible during Phase 1 A of Virginia’s COVID-19 Vaccination Campaign. There is no
need to answer any further questions.
If you answered no, please continue to question 3.
Question 3
Do you currently work or volunteer in any of the following?
Police, Fire, or Hazmat
Corrections and homeless shelter worker
Childcare/Pre-K-12 teacher or staff
Food and Agriculture worker, including veterinarian
Manufacturing worker
Grocery store worker
Public transit worker
Mail carriers (USPS or private)
Official needed to maintain continuity of government
If you answered yes to working or volunteering in any of the options listed, you
are eligible during Phase 1b of Virginia’s COVID-19 Vaccination Campaign. There is
no need to answer any further questions.
If you answered no, please continue to question 4.
Question 4
Are you currently living in a correctional facility, homeless shelter or migrant labor
camp?
If you answered yes to this question, you are eligible for Phase 1b of Virginia’s
COVID-19 Vaccination Campaign. There is no need to answer any further
questions.
If you answered no, please continue to question 5.
Question 5:
Do you have any of these medical conditions? Please note that if you have
questions about your medical condition, contact your healthcare provider.
Moderate to severe asthma
Cancer
Cerebrovascular Disease
Chronic kidney disease
COPD which stands for chronic obstructive pulmonary disease
Cystic fibrosis
Down Syndrome
Heart Conditions such as heart failure, coronary artery disease,
cardiomyopathies also known as heart muscle disease
Hypertension or High Blood Pressure
Immunocompromised state, for example weakened immune system from
blood, bone marrow or solid organ transplant, immune deficiencies, HIV,
use of corticosteroids, or use of other immune weakening medicines
Liver Disease
Neurologic conditions
Severe Obesity meaning your body mass index is greater than 40 kilograms
per meter squared
Obesity meaning that your body mass index is equal to or greater than 30
kilograms per meter squared but not greater than 40 kilograms per meter
squared
Overweight meaning that your body mass index is equal to or greater than
20 kilograms per meter squared but less than 30 kilograms per meter
squared
Pregnancy
Pulmonary fibrosis, having damaged or scarred lung tissues
Sickle cell disease
Smoking
Thalassemia
Type 1 or Type 2 Diabetes mellitus
If you answered yes to having at least one of the above listed medical
conditions you are eligible for Phase 1b of Virginia’s COVID-19 Vaccination
Campaign. There is no need to answer any further questions.
If you answered no, please continue to question 6.
Question 6
Do you currently work or volunteer in any of the following fields?
Energy
Waste and wastewater
Housing construction
Food service
Transportation and logistics
Institutions of Higher Education faculty or staff
Finance worker
Information Technology or Communications
Media
Legal Services
Public Safety, engineers
Public Health
If you answered yes to any of the above occupations, you are eligible during
Phase 1c of Virginia’s COVID-19 Vaccination Campaign.
If you answered no to all of the questions asked in this questionnaire, you can get
the COVID-19 vaccine in a future phase when it is offered to all people your age.
